Referrals and Intake
From Referral to Treatment
Clients are referred to Exceptional Wellness from many sources (family, insurance company, doctor's office, school district, hospital, etc.) The person making the referral provides the client's name, date of birth, any other relevant information, and the phone number where we can contact the referral. That information is then passed over to the Intake Department where one of our Intake Specialists will call the client and conduct an intake over the phone. If the client is a minor, the intake will be conducted with the client's parent or legal guardian. After the intake has been completed, the Intake Specialist will complete an Adult Needs and Strengths Assessment (ANSA), or Child and Adolescent Needs and Strengths assessment (CANS) and the initial assessment. The initial assessment provides an overall picture of the client and the issues s/he is experiencing.
Once the ANSA/CANS and the initial assessment have been completed, then the assigned Licensed Professional Counselor or Psychologist will review the information and create a customized treatment plan for the client which will have designated goals based on the issues the client is experiencing. The treatment plan will guide the client's treatment and is utilized by Case Managers and Therapists to guide the treatment of the client. If the client also needs Psychiatry services, s/he will also be referred to the Psychiatrist for medication management and an appointment will be set within 48 hours so that the doctor can assess the client.
Each month, Case Managers will review each client on their caseload with a supervisor to discuss the progress of the client. If the client is a minor their treatment plan is reviewed and updated every 90 days; if the client is an adult their treatment plan is reviewed and updated every 180 days.
